Special Topic Introduction
Background and Rationale:
Diabetic cardiovascular disease is one of the most common and fatal complications of diabetes. Chronic hyperglycemia induces metabolic disorders, persistent inflammation, oxidative stress, and endothelial dysfunction, accelerating myocardial injury and vascular remodeling. In recent years, studies on molecular mechanisms have revealed multiple key signaling pathways and therapeutic targets, providing new directions for multi-target treatments such as anti-inflammatory, antioxidant, metabolic regulation, and stem cell-based therapies.
Objectives:
● To synthesize and disseminate current mechanistic insights into how diabetes drives cardiovascular pathology.
● To highlight translational advances and emerging therapeutic strategies that target diabetes‑related cardiovascular processes.
● To promote multidisciplinary research and methodological innovation that accelerate bench‑to‑bedside translation.
● To identify and critically evaluate biomarkers, imaging modalities, and risk‑stratification tools that improve early detection, prognostication, and patient selection for targeted interventions.
Topics Covered:
● Molecular and cellular mechanisms linking diabetes to cardiovascular disease.
● Endothelial dysfunction, vascular inflammation, immune cell-driven processes, and thrombosis in diabetic atherosclerosis and microvascular disease.
● Therapeutic advances, including glucose‑lowering agents with cardiovascular benefits, novel metabolic modulators, lipid‑lowering and anti‑inflammatory strategies, antithrombotic approaches, and cardioprotective drugs.
● Regenerative and gene‑based therapies, such as stem/progenitor cell therapies, gene editing, RNA therapeutics, and tissue engineering for myocardial repair.
● Precision medicine, pharmacogenomics, and patient‑centric approaches to tailor cardiovascular prevention and therapy in diabetes.
● Clinical trial design, real‑world evidence, implementation science, cost‑effectiveness analyses, and guideline implications for managing cardiovascular disease in patients with diabetes.
● Health disparities, socioeconomic determinants, and population‑level strategies to reduce the global burden of diabetic cardiovascular disease.
● Sex and gender differences in diabetic cardiovascular disease: underlying mechanisms and therapeutic implications.
● The role of hepatic and renal health in modulating diabetic cardiovascular risk.
